Lengthening methods
External fixation (Ilizarov)
The Ilizarov method uses an external frame throughout both the lengthening and consolidation phases. Even with LON and LATN available, it remains essential for complex deformity. The institute now pairs it with a computer-assisted correction system (Ortho-SUV, a Stewart platform), correcting multi-plane deformity in a single operation — the largest case volume of its kind in Korea.
Who it suits
- Complex, multi-plane deformity
- Leg length discrepancy combined with deformity
- Cases where a nail is impossible — infection or grossly abnormal bone shape
Technique highlights
- Simultaneous multi-plane correction with the Ortho-SUV computer system
- Fine adjustment mid-treatment without repeat surgery
- Usable even in previously infected or previously operated bone
Surgical steps
- Deformity analysis on weight-bearing radiographs, computed in software.
- Osteotomy and ring-frame application.
- Daily strut adjustments following the software-generated schedule.
- Frame removal once the regenerate is strong, followed by full rehabilitation.
Recovery & aftercare
- The frame stays on longer than in LON/LATN — through the entire consolidation phase.
- Daily pin-site care is required to prevent infection.
- You must stay in Korea for the whole lengthening and follow-up period; the international team arranges accommodation, interpretation and the physiotherapy schedule.
Risks & things to consider
- Bone lengthening is major surgery. Outcome and recovery time depend on bone quality, age, weight and how disciplined the patient is with rehabilitation, so identical results cannot be guaranteed.
- Daily physiotherapy is mandatory through both the lengthening and consolidation phases. Skipping it risks joint stiffness, muscle contracture or delayed bone healing.
- Disclose all medical conditions, regular medication, previous bone surgery and drug allergies at the consultation, and stop smoking before and after surgery as instructed — nicotine measurably slows bone healing.
- Pin scars and day-to-day inconvenience while the frame is worn.
Frequently asked questions
Is Ilizarov still needed now that motorised nails exist?
Yes, in many situations — complex multi-plane deformity, previously infected bone, or bone that cannot take a nail — because the frame allows fine correction throughout treatment.
